Certificate of Insurance

IMPORTANT: Each participant must email a current and valid certificate of insurance to info@elburn.com by 07/31/2023 with the holder and additional insured listed as Elburn Lions Community Park, Inc. & Elburn Lions Club, 500 Filmore St, Elburn, IL 60119

The Elburn Chamber Health and Wellness Fair has been scheduled for September 17th 2022.

We are looking for all types of businesses that can help people achieve a healthy lifestyle!

Examples: Goat Yoga, Gym Memberships, Finance, Insurance- health and life, Pet Adoption, Therapeutic Riding and more

Certificate of Insurance

IMPORTANT: Each participant must have a Certificate of Insurance (COI) listing the Elburn Lions Club on file with the Elburn Chamber of Commerce. COI’s may be emailed to the chamber office at info@elburn.com. Payment can be made online or by check. Checks should be payable to the Elburn Chamber of Commerce and can be mailed to the chamber office at 100 S. Main Street, Unit 1S Elburn, IL 60119.
